Payment Information
A minimum $100 deposit is required before your TEAM will be added to the league. 
Once your Registration Form has been received and processed, you will receive an Email or Phone Call with instructions to log into our online system to process your TEAM Deposit and/or TEAM Payment. 
Team balances are due by your first game and can be paid online at .

 All Leagues Beginning After October 1, 2008 Require A Valid Player's Card

INDIVIDUAL Registrations will be added to our Waiting List for this League. 
If we are able to place you on a "House Team or an Existing Team, you will receive an Email or Phone Call
with instructions to log into our online system to process your INDIVIDUAL payment.

 
 Roster Information

Team deposits must be paid within 24 hours of receiving your registration confirmation.  Team  balances are due by the first game.  All captains and players should read and  understand the   Sports Connection Rules & Policies and the League Rules of your specific  league.  All  players  must have a current Liability Waiver on file with Sports Connection  before playing and  have an Email address on file to access our online program.


Consent Form
All participants must have a Signed Release and Waiver Of Liability and Indemnity Agreement and Consent Form on file with Sports Connection before they will be allowed to participate in any Leagues or Other Activities associated with the Sports Connection, LLC.  Participants under 18 years of age must have a parent or guardian sign the Sports Connection, LLC Release and Waiver Of Liability and Indemnity Agreement.

I,    (self/parent/guardian), desire for my (self/son/daughter) to participate in one of Sports Connection, LLC's or Charlotte Sports Center, LLC' leagues or other activities and have read and agree to the terms and conditions listed below regarding the Release and Waiver of Liability and Indemnity Agreement. I understand that playing in a sports league or other activity involves physical contact between players and I assume the risks associated with (my/his/her) participation. (I/My child) (have/has) no known medical conditions which prohibit participation.  I agree to hold harmless and release Sports Connection, LLC, Charlotte Sports Center, LLC and their agents and sponsors from legal liability resulting from any injuries sustained during participation in these activities. I understand Sports Connection, LLC and Charlotte Sports Center, LLC cannot be responsible for loss or damage to (my/my child's) personal possessions. You may use (my/my child's) photograph in future promotions.
